Praxis Score Reporting

Students in the master’s program in Speech Language Pathology are required to report official Praxis scores to the College as both a report recipient using code 0970 and an attending institution using code 1702. At the time of testing, you may arrange for you scores to be sent to up to four institutions/agencies you selected during the registration process. We recommend that you send your scores to ASHA and the licensure board for the state(s) in which you intend to work in addition to the College.

Use the list of codes to select your Attending Institution and Designated Score Recipients when you register. These codes are for candidates taking the Audiology or Speech-Language Pathology tests. The Attending Institution section requires codes preceded by an A or the institution(s) where you received training relevant to the test. The Designated Score Recipients section requires codes preceded by an R for the institution(s) where you want your score report sent. RA codes may be used in either section.

Under each state, state departments and agencies are listed first and bolded, followed by speech-language pathology and audiology departments of colleges and universities. Codes are added and revised throughout the year. If you are not able to find a code you are looking for, please contact ETS Customer Service to see if one was assigned after our web publication date.

Example 1 Saint Mary’s master’s student taking the Praxis who plans to work in Indiana

1. List Saint Mary’s College as both a report recipient using code 0970 and an attending institution using code 1702

2. Also send your official Praxis scores to the following Designated Score Report Recipients
R7240 IN SLP/Aud Board for Indiana
R5031 American Speech-Language- Hearing Assoc Cert
RA0970 St. Mary’s College IN-SLP
*Note one additional Designated Score Recipient report is included in your registration.

Example 2 Saint Mary’s master’s student taking the Praxis who plans to work in Illinois

1. List Saint Mary’s College as both a report recipient using code 0970 and an attending institution using code 1702

2. Also send your official Praxis scores to the following Designated Score Report Recipients

Example 3 Saint Mary’s master’s student taking the Praxis may work in Indiana or Michigan

If at a later time you need to send official Praxis scores to an entity, you can order an additional report from ETS at an additional fee.
